Antidepressants: The Most Commonly Prescribed Class

SSRIs — Selective Serotonin Reuptake Inhibitors

Examples: Sertraline (Zoloft), Fluoxetine (Prozac), Escitalopram (Lexapro), Paroxetine (Paxil), Citalopram (Celexa). SSRIs are the first-line treatment for depression and most anxiety disorders. They work by increasing serotonin availability in the brain. They typically take 4–6 weeks to reach full effectiveness. Common side effects include nausea (usually temporary), insomnia or drowsiness, and sexual side effects.

SNRIs — Serotonin-Norepinephrine Reuptake Inhibitors

Examples: Venlafaxine (Effexor), Duloxetine (Cymbalta), Desvenlafaxine (Pristiq). SNRIs affect both serotonin and norepinephrine. They are effective for depression, anxiety, and also have FDA approval for chronic pain conditions like fibromyalgia and diabetic neuropathy. Duloxetine is particularly useful for patients with both depression and chronic pain.

Atypical Antidepressants

Examples: Bupropion (Wellbutrin), Mirtazapine (Remeron), Trazodone. Bupropion is unique — it has no sexual side effects and is also FDA-approved for smoking cessation. Mirtazapine is often used when weight gain and improved sleep are desired. Trazodone is frequently used at low doses as a sleep aid.

Anti-Anxiety Medications

Benzodiazepines (Short-Term Use)

Examples: Alprazolam (Xanax), Lorazepam (Ativan), Clonazepam (Klonopin), Diazepam (Valium). Benzodiazepines provide rapid relief of anxiety symptoms but are intended for short-term use due to the risk of dependence and tolerance. Mood Stabilizers for Bipolar Disorder

  • Lithium — the gold standard for bipolar disorder; requires regular blood level monitoring
  • Valproate (Depakote) — also used for seizures; requires liver function monitoring
  • Lamotrigine (Lamictal) — particularly effective for bipolar depression; requires slow dose titration
  • Atypical antipsychotics (Quetiapine, Olanzapine, Aripiprazole) — used for both mania and depression in bipolar disorder

Critical Points About Psychiatric Medications

  • Never stop psychiatric medications abruptly without medical guidance — this can cause serious withdrawal or relapse
  • Most antidepressants take 4–6 weeks for full effect — give them time before concluding they don't work
  • Side effects are often temporary and improve after the first 1–2 weeks
  • Alcohol significantly interacts with most psychiatric medications
  • Inform your pharmacist of ALL medications including supplements — many interact with psychiatric drugs
