Metformin: The Foundation of Type 2 Diabetes Treatment

Metformin (Glucophage) is the first-line medication for type 2 diabetes in most patients. It works primarily by reducing glucose production in the liver and improving insulin sensitivity. It does not cause hypoglycemia (low blood sugar) when used alone, is weight-neutral or causes modest weight loss, and has decades of safety data. The most common side effects are GI (nausea, diarrhea) — taking with food and starting at a low dose minimizes these.

Insulin: Types and When They're Used

Rapid-Acting Insulin (Bolus)

Examples: Lispro (Humalog), Aspart (NovoLog), Glulisine (Apidra), and ultra-rapid Fiasp. These are taken at mealtimes to cover the glucose rise from food. They start working in 10–15 minutes and last 3–5 hours. Newer ultra-rapid formulations (Fiasp, Lyumjev) can be taken at the start of or even after a meal.

Long-Acting Insulin (Basal)

Examples: Glargine (Lantus, Basaglar, Toujeo), Detemir (Levemir), Degludec (Tresiba). These provide a steady background level of insulin over 24 hours (or longer for Tresiba). They are typically taken once daily at the same time each day. They should NOT be mixed with other insulins.

Modern Diabetes Medications: Game-Changers in Treatment

GLP-1 Receptor Agonists

Examples: Semaglutide (Ozempic, Wegovy, Rybelsus), Liraglutide (Victoza, Saxenda), Dulaglutide (Trulicity), Exenatide (Byetta, Bydureon). GLP-1 agonists stimulate insulin release, suppress glucagon, slow gastric emptying, and reduce appetite. They cause significant weight loss (5–15% of body weight) and have proven cardiovascular and kidney-protective benefits. Ozempic and Trulicity are weekly injections; Rybelsus is an oral tablet.

SGLT2 Inhibitors

Examples: Empagliflozin (Jardiance), Dapagliflozin (Farxiga), Canagliflozin (Invokana). These medications cause the kidneys to excrete excess glucose in the urine, lowering blood sugar. They also reduce blood pressure, cause weight loss, and have remarkable cardiovascular and kidney-protective effects. They increase the risk of urinary tract infections and genital yeast infections.

Insulin Storage and Handling

  • Unopened insulin: store in refrigerator (36–46°F) until expiration date
  • Opened/in-use insulin: can be kept at room temperature for 28–30 days (varies by product)
  • Never freeze insulin — freezing destroys it
  • Protect from direct sunlight and heat above 77°F
  • Inspect before each use — discard if cloudy (for clear insulins) or if particles are present
  • Rotate injection sites to prevent lipohypertrophy (fatty lumps)
