Nevada has one of the highest rates of type 2 diabetes in the nation — approximately 11.4% of adults are diagnosed, with many more undiagnosed. In Las Vegas, the combination of sedentary lifestyles, high-calorie dining culture, and limited access to fresh produce creates a challenging environment for diabetes management. At Advantixx Pharmacy, diabetes care is one of our most important clinical missions.

Understanding Type 2 Diabetes: What's Actually Happening

Type 2 diabetes develops when the body becomes resistant to insulin and/or the pancreas can't produce enough insulin to maintain normal blood sugar levels. Unlike type 1 diabetes (an autoimmune condition), type 2 is strongly influenced by lifestyle factors — though genetics play a significant role. The result is chronically elevated blood glucose that, over time, damages blood vessels and nerves throughout the body.

The A1C: Your Most Important Diabetes Number

The A1C test measures your average blood sugar over the past 2–3 months. For most adults with type 2 diabetes, the target A1C is below 7.0%. Every 1% reduction in A1C reduces the risk of microvascular complications (eye, kidney, nerve damage) by approximately 37%. At Advantixx Pharmacy, we track your A1C trends and work with your physician to adjust your regimen when targets aren't being met.

  • A1C below 5.7% — Normal
  • A1C 5.7–6.4% — Prediabetes
  • A1C 6.5% or higher — Diabetes diagnosis
  • A1C below 7.0% — Target for most adults with diabetes
  • A1C below 8.0% — Acceptable target for elderly or those with complex health issues

First-Line Medications: Metformin and Beyond

Metformin remains the cornerstone of type 2 diabetes treatment for most patients. It's inexpensive, well-studied, weight-neutral, and does not cause hypoglycemia when used alone. However, many patients require additional medications as diabetes progresses. The choice of second-line agent is now guided by individual patient factors — particularly cardiovascular disease, kidney disease, and weight.

For Patients with Cardiovascular Disease or High CV Risk

GLP-1 receptor agonists (Ozempic/semaglutide, Trulicity/dulaglutide, Victoza/liraglutide) and SGLT2 inhibitors (Jardiance/empagliflozin, Farxiga/dapagliflozin) have proven cardiovascular benefits and are now preferred second-line agents for patients with established heart disease or high cardiovascular risk.

For Patients with Chronic Kidney Disease

SGLT2 inhibitors — particularly Jardiance and Farxiga — have demonstrated remarkable kidney-protective effects in clinical trials, slowing the progression of diabetic kidney disease. They are now recommended by major nephrology and diabetes guidelines for patients with CKD and diabetes.

For Patients Who Need Weight Loss

GLP-1 agonists, particularly semaglutide (Ozempic/Wegovy) and tirzepatide (Mounjaro/Zepbound), produce significant weight loss — 10–20% of body weight in clinical trials. For obese patients with type 2 diabetes, these medications can be transformative, sometimes achieving near-remission of diabetes.

Preventing the Big Three Complications

Diabetic Neuropathy (Nerve Damage)

Peripheral neuropathy — burning, tingling, or numbness in the feet and hands — affects up to 50% of people with diabetes. Tight blood sugar control is the most effective prevention. Treatment options include gabapentin, pregabalin, duloxetine, and topical agents. Daily foot inspections are critical — neuropathy can mask injuries that lead to serious infections.

Diabetic Nephropathy (Kidney Disease)

Diabetes is the leading cause of kidney failure in the US. ACE inhibitors or ARBs are recommended for all diabetic patients with any sign of kidney involvement. SGLT2 inhibitors provide additional kidney protection. Regular monitoring of kidney function (eGFR, urine albumin) is essential.

Diabetic Retinopathy (Eye Disease)

Diabetes is the leading cause of new blindness in working-age adults. Annual dilated eye exams are essential. Tight blood sugar and blood pressure control are the primary preventive strategies. Anti-VEGF injections are used to treat advanced retinopathy.

Lifestyle: The Foundation of Diabetes Management

  • Aim for 150 minutes of moderate exercise per week — even walking significantly improves insulin sensitivity
  • Focus on low-glycemic foods: vegetables, legumes, whole grains, lean proteins
  • Limit refined carbohydrates, sugary beverages, and processed foods
  • Maintain a healthy weight — even 5–10% weight loss significantly improves blood sugar control
  • Monitor blood sugar regularly and keep a log to share with your healthcare team
  • Prioritize sleep — poor sleep dramatically worsens insulin resistance
