Chronic pain — defined as pain lasting more than 3 months — affects an estimated 50 million adults in the United States and is one of the most common reasons people seek medical care. The Pain Medication Ladder: From Mild to Severe

Step 1: Non-Opioid Analgesics

Acetaminophen (Tylenol) is the first-line treatment for mild to moderate pain. It is safe when used as directed (maximum 3,000–4,000 mg/day for most adults) but can cause serious liver damage in overdose or with chronic alcohol use. It does not reduce inflammation.

Step 2: NSAIDs — Non-Steroidal Anti-Inflammatory Drugs

NSAIDs include ibuprofen (Advil, Motrin), naproxen (Aleve), celecoxib (Celebrex), and prescription-strength versions. They reduce pain AND inflammation, making them particularly effective for arthritis, muscle injuries, and inflammatory conditions. Important risks include GI bleeding (take with food), kidney damage (especially in dehydrated or elderly patients), and cardiovascular effects with long-term use.

Step 3: Neuropathic Pain Agents

For nerve pain (burning, shooting, tingling), different medications are needed. Gabapentin (Neurontin) and Pregabalin (Lyrica) are anticonvulsants that are highly effective for diabetic neuropathy, post-herpetic neuralgia, and fibromyalgia. Duloxetine (Cymbalta) is an SNRI antidepressant with FDA approval for several chronic pain conditions. Tricyclic antidepressants (amitriptyline, nortriptyline) at low doses are also effective for neuropathic pain.

Step 4: Muscle Relaxants

Cyclobenzaprine (Flexeril), Methocarbamol (Robaxin), Tizanidine (Zanaflex), and Baclofen are used for muscle spasm and musculoskeletal pain. They are generally intended for short-term use. Sedation is a common side effect — do not drive or operate machinery.

Topical Pain Medications: Effective with Fewer Side Effects

  • Diclofenac gel (Voltaren) — topical NSAID for localized joint pain; minimal systemic absorption
  • Lidocaine patches (Lidoderm) — for post-herpetic neuralgia and localized neuropathic pain
  • Capsaicin cream — depletes substance P; effective for arthritis and neuropathy

Opioid Medications: Important Considerations

Opioids (hydrocodone, oxycodone, morphine, tramadol) are reserved for moderate to severe pain that has not responded to other treatments. They carry significant risks including dependence, tolerance, constipation, respiratory depression, and overdose.
