Anticoagulants — commonly called blood thinners — are medications that prevent blood clots from forming or growing. They are prescribed for conditions including atrial fibrillation (AFib), deep vein thrombosis (DVT), pulmonary embolism (PE), mechanical heart valves, and stroke prevention. Warfarin (Coumadin): The Classic Blood Thinner

Warfarin has been used for over 60 years and remains the only anticoagulant approved for patients with mechanical heart valves. It works by blocking vitamin K-dependent clotting factors. The major challenge with warfarin is its narrow therapeutic window — too little and clots can form; too much and dangerous bleeding can occur. This is why regular INR (International Normalized Ratio) blood tests are required.

  • Target INR for most conditions: 2.0–3.0
  • Target INR for mechanical heart valves: 2.5–3.5
  • INR testing frequency: weekly when starting, monthly when stable
  • Dozens of drug interactions — always inform your pharmacist of any new medication
  • Vitamin K in food affects warfarin — consistency is key (don't avoid leafy greens, just eat them consistently)

Direct Oral Anticoagulants (DOACs): The Modern Alternative

DOACs have largely replaced warfarin for most indications because they don't require routine blood monitoring, have fewer food interactions, and have more predictable effects. However, they are more expensive and some lack readily available reversal agents.

Apixaban (Eliquis)

Eliquis is a Factor Xa inhibitor taken twice daily. It has the lowest bleeding risk among DOACs in clinical trials and is FDA-approved for AFib, DVT/PE treatment and prevention, and post-surgical clot prevention. It should not be crushed (but can be dissolved in water for patients who can't swallow tablets).

Rivaroxaban (Xarelto)

Xarelto is also a Factor Xa inhibitor but is taken once daily (with the evening meal for AFib and DVT/PE). The once-daily dosing improves adherence. It has similar efficacy to Eliquis but slightly higher GI bleeding rates in some studies.

Dabigatran (Pradaxa)

Pradaxa is a direct thrombin inhibitor taken twice daily. It has a reversal agent (Praxbind/idarucizumab) available. It must be stored in its original bottle (not a pill organizer) and discarded 4 months after opening. It can cause significant GI side effects.

Critical Drug Interactions with Blood Thinners

  • NSAIDs (ibuprofen, naproxen) — significantly increase bleeding risk with all anticoagulants
  • Aspirin — increases bleeding risk; only use if specifically prescribed alongside anticoagulant
  • Antibiotics — many interact with warfarin; always inform your pharmacist
  • Antifungals (fluconazole) — dramatically increase warfarin effect
  • St. John's Wort — reduces effectiveness of warfarin and DOACs
  • Rifampin — significantly reduces DOAC levels

What to Do If You Miss a Dose
