Arthritis is not a single disease — it's an umbrella term for over 100 conditions affecting joints, surrounding tissues, and other connective tissues. The two most common forms are osteoarthritis (OA), a degenerative "wear and tear" condition, and rheumatoid arthritis (RA), an autoimmune disease. At Advantixx Pharmacy in Las Vegas, we manage medications for both — and the treatment approaches are dramatically different.

Osteoarthritis: Managing Pain and Preserving Function

Osteoarthritis results from the breakdown of cartilage in joints — most commonly the knees, hips, hands, and spine. There are no disease-modifying treatments for OA; management focuses on pain relief and maintaining function.

Acetaminophen (Tylenol)

Acetaminophen is often the first-line recommendation for mild OA pain. It is safer than NSAIDs for long-term use, particularly in elderly patients or those with kidney or GI issues. Maximum dose: 3,000–4,000mg/day. Caution with alcohol use.

NSAIDs: Oral and Topical

NSAIDs (ibuprofen, naproxen, celecoxib) are more effective than acetaminophen for OA pain because they reduce inflammation. Topical diclofenac gel (Voltaren) is highly effective for knee and hand OA with minimal systemic absorption — making it safer for patients with GI or kidney concerns. Celecoxib (Celebrex) is a COX-2 selective NSAID with lower GI bleeding risk.

Rheumatoid Arthritis: Disease-Modifying Treatment

Unlike OA, rheumatoid arthritis is driven by immune system dysfunction that attacks the joints. Without treatment, RA causes progressive joint destruction and disability. The goal of treatment is remission — not just symptom control.

Conventional DMARDs

Methotrexate is the anchor of RA treatment — it's inexpensive, effective, and has decades of safety data. It's taken weekly (not daily) and requires folic acid supplementation to reduce side effects. Regular monitoring of liver function and blood counts is required. Other conventional DMARDs include hydroxychloroquine (Plaquenil), sulfasalazine, and leflunomide.

Biologic DMARDs

Biologics are targeted therapies that block specific components of the immune response driving RA. They are used when conventional DMARDs are insufficient. TNF inhibitors (Humira/adalimumab, Enbrel/etanercept, Remicade/infliximab) were the first biologics approved for RA and remain widely used. Newer biologics target different pathways: IL-6 (Actemra/tocilizumab), B-cells (Rituxan/rituximab), and T-cell activation (Orencia/abatacept).

JAK Inhibitors: The Newest Class

JAK inhibitors (tofacitinib/Xeljanz, baricitinib/Olumiant, upadacitinib/Rinvoq) are oral targeted therapies — a significant advantage over injectable biologics. They are highly effective but carry important safety considerations including increased infection risk, cardiovascular events, and malignancy risk with long-term use.

Gout: A Special Form of Arthritis

Gout is caused by uric acid crystal deposition in joints, causing sudden, severe attacks of pain and swelling — most commonly in the big toe. Acute attacks are treated with NSAIDs, colchicine, or corticosteroids. Long-term prevention uses urate-lowering therapy: allopurinol (first-line) or febuxostat. Dietary modifications (reducing red meat, alcohol, high-fructose corn syrup) are also important.

Important Monitoring for Arthritis Medications

  • Methotrexate: CBC and liver function tests every 3 months
  • Biologics: TB screening before starting; infection monitoring throughout
  • NSAIDs: kidney function and blood pressure monitoring with long-term use
  • Hydroxychloroquine: annual eye exam (rare retinal toxicity with long-term use)
  • JAK inhibitors: lipid panel, CBC, and infection monitoring
